If a participant is under 18, a parent or legal guardian must communicate with MMHP, provide the participant’s information, manage the participant’s account, and complete all required agreements and authorizations. Clients under 18 must have a parent or legal guardian authorize participation and complete required contracts, waivers, and health forms unless applicable law provides otherwise.
